day of January in the year aforesaid being unployed by oneGubbins of the said Parish of Allhallows the Great
in the Ward aforesaid in London aforesaid Carpenter
in the Yard of the saidGubbins there situate and being
then and there ever unto a certain pile of Deals then standing and resting on a certain arch erected and built in
the said Yard for that purpose it so happened that the said Arch whereon the said pile of Deals on then and there rested
and stood as aforesaid did by the weight and pressence of the said Pile of Deals then and there give way and fall down
And the said Pile of Deals so standing and resting as the said Arche by reason and means of the giving way
and falling down of the said Arch as aforesaid then and there accidentally casually and by misfortune did
full upon and get the said

there receive divers mortal wounds and bruises in and upon the head face back sides arms legs and thighs of
