aforesaid who being now here duly chosen sworn and charged
to inquire for our said Lord the King when how and in what manner the said

came to her death say upon their oath
that a certain person whose name to the Jurors aforesaid is as yet unknown in the twentieth day of September in the twenty eighth
year aforesaid at the Parish of Saint Sepulcher in the County of Middlesex
of being driving a certain Coach drawn by four Horses along and
through a certain public street and common Highway there called Saint John's Street
and the said

then being in the said
public Street
and common Highway aforesaid It is happened that the said

accidentally casually and by misfortune fell to
and against the Ground there and near to the said Coach and while she said Coach was then and there going and he going along and
through the said public street and common Highway as aforesaid By means whereof the near Bender Wheel of the said Coach did then and
there accidentally casually and by misfortune go upon and pass over the Body of her the said

did then and there receive divers mortal wounds and Bruises in and upon the Back sides arms Legs and Body of

in the twenty eighth year aforesaid as well as the said Parish of Saint Sepulchre in the said County of Middlesex

Sepulchre in the Ward of Farringdon
without in London
aforesaid did languish and langushing did live On which said twentieth day of
September in the twenty eighth year aforesaid at the Parish last aforesaid in the Ward aforesaid in London

said mortal wounds and bruises did die And so the Jurors aforesaid upon their oath aforesaid do say that the said

misfortune was accidentally killed And that the near hinder Wheel of the said Coach was moving to the death of the said
