October in the year aforesaid being in the landing place of the two pair of Stairs in the dwelling house
of oneDavidson of Clement Lane
in the Parish and Ward and aforesaid in London aforesaid Gentleman
and near unto the stair case there he the said Henry Horsley accidentally casually and by misfortune
with great force and violence did fall from the said landing place down the said Stairs By means whereof

back of him the said
