Informations taken this Twenty first
day of March 1776 at the Parish of St. George
Hanr. Square within the Liberty of
Westmr. in the County of Midsex upon
an Inquisition touching the death of
John Smith< no role > lying dead in the said Parish
Liberty and County

George Charters< no role > House Surgeon at St. George's
Hospital in the Parish of St. George Hanr. Square
Westmr. on his Oath Saith, That on Monday Morning
last (March 18th.) John Smith< no role > the Deced was brought
to said Hospital as an Accident being then insensible
Says that he had before been sent for to bleed
the Deced in Hyde Park who he was informed
had a fall from a Horse and that the Horse
afterwards hadafterwardsfell on down upon
him, Says that no external hurt appeared
upon the Deced, Says that he Deced continued
insensible, until about Ten o'Clock the same
Night when he died in said Hospital, Says
that he opend the Deced's Head & Body on
the 19th. Instant and found a Vessel Ruptured
in the Head which he believes to be the cause
of Deced's Death, and thinks that the Vessel
in the Head might be Ruptured by felling
off the Horse as abovementioned.

William Pettkin< no role > Servant to Mr. Smith a
Brewer in Park Lane in the Parish of St. George
Hanr. Square on his Oath saith, that on Monday
Morning last (March. 18th) Dept. was in his Master's
Brewhouse and looking through a Window into
Hyde Park he saw the Deced riding upon a Blank
Horse in Hyde Park That the Horse appeared to
run away with the Deced for about two Minutes
That the Deced with the Runs sharply pulled
the Horse's Head to the left and that the Horse
immediately
